Vaccinations

The state of Mississippi requires schools to ensure students have vaccinations before enrollment.

While there are some exceptions, the required vaccinations are as follows:

Required for all students: DTaP - 5 shots (or 4, if the fourth was was received on or after the child’s fourth birthday)

Required for all students: Polio - 4 shots (or 3, if the third was received on or after the child’s fourth birthday and six months after shot two)

Required for all students: Hep B - 3 shots

Required for all students: MMR - 2 shots

Required for all students: Varicella - 2 shots

Required for 7th and 8th grade students: Tdap - 1 shot

If your child does not yet have all required vaccinations

Please contact your primary health care provider or local health department for additional information.

After getting updated shots, please ask your doctor for a 121 form, and bring the 121 to our school.

If your child already has all required vaccinations

You simply need to submit the 121 form!

The state now allows you to access shot records / 121 forms online

STEP ONE: Create an MyIR account if you haven’t already

  1. Click here.

  2. Choose Register

  3. Select Mississippi

  4. Enter your name, e-mail address, and create a password

  5. Choose Sign Up, then verify your account by phone or e-mail.

STEP TWO: Find the 121 form for your child

Once you create your account and are logged in:

  1. Choose Find My Records.

  2. Fill out the required personal information to help the system locate your child’s 121 form.

  3. If your information matches an entry in the immunization registry, choose Take Me to My Account to download the 121 form.

  4. If your information does not exactly match an official immunization record, use the green chat bubble on the bottom right of the screen to provide extra details to help locate your child’s records.

STEP THREE: Access your 121 form

Once you have located your account, check the Documents section for an immunization history or Form 121 to download or print.

STEP FOUR: Submit the 121 form

You can bring in a paper copy of your 121 form to our school, or email it to reception@midtownpublic.org
